Involved Persons
This form asks you about other
persons involved in some of your usual activities (i.e., those activities you
indicate in the Activity Selection
page).
There are two questions for each
activity displayed. Depending on the activity, you may need to answer either
one of them or both of them (You will not see the one you don't need to
answer).
Question 1:
If other persons occasionally
do this activity for you (so you can do something else), please check them from
the following list.
For example, I usually pick up
my son from school. But sometimes my wife could do that for me. In this case my
wife can do this activity FOR me. I will check the check box in front of her
name.
Question 2:
If you usually do this
activity with other persons, check them in the following list.
For example, I always go to
movies WITH my wife. I will check the box in front of her name.
