Member Information
This page is intended to collect the
personal information of each household member. This page should be filled out
by the household member whose name is indicated on top of the page. Parents
please answer these questions for minors. When answering for you children,
please remember all the following questions are addressed to him/her, not the
parents. This form will be the only one you need to
fill out for your children. After this form, you can click [Next>] to skip
all the questions (answer Yes to all the warning prompts).

each question:
Relation to Head of Household: What is your relationship to heads of
the household?
(Heads of household: the couple who are the
center of the household, making most of the financial and management decisions
in the household)
· Head (Male): If you are the male head of
the household or if you are a single male, select this option.
· Head (Female): If you are the female head
of the household or if you are a single female, select this option.
· Child: If you are a child of the household
heads, select this option.
· Sibling: If you are a brother of sister
of the household heads, select this option.
· Parent: If you are a parent of the
household heads,
· Relative: If you are a relative (other
than all of the above) of the household heads, select this option.
· Unrelated: If you have no relationship to
the household heads, select this.
Educational Level: What is the highest degree you
currently have in education? (It
should be the highest degree you already have, NOT the degree you are currently
working for.)
Employment Status: What is your employment status (e.g.,
full time, part time, unemployed ...) ?
Hours worked in a typical
week: Enter into the
text box the number of hours you work in a week with no holidays.
Occupation: What is your occupation?
Student Level: What kind of school are you currently
attending? (This is the degree
you are currently pursuing.)
School name: Enter into the text box name of your
current school .
School city: Enter name of the city where your school
is located.
